Please fill out the Registration form below and mail it in with your check. Registration of the day of the race will begin at 12:30p.m. at Bartness Park
Awards: Cash and/or Product Prizes for Top 5's in each grouping
Entry Fee: Pre-Registration (before August 7th) $10.00 Day of Tournament $15.00

Amateurs (Beginners) -First Timers and those who have played only a few times
Advanced - Those who have played at least a full season or have thrown on a league
Age Group: (Circle One) 5-12 13-17 18 and up
Gender: (Circle One) Male Female
I understand that I am participating in this event and agree to assume any and all liability for accidents or injuries, whether or not the Flekkefest Committee, Elbow Lake, MN, or City of Elbow Lake were negligent. I further agree to hold said committee and city harmless and will indemnify them for any loss or amounts they must pay by reason of accident or injury to me.
